The LS25/26 Primary Care Network (PCN) is a group of local GP practices working together to improve the health and care of their community by providing more proactive, personalised, and joined‑up health services. As part of the wider NHS initiative, PCNs bring together GP surgeries and other healthcare professionals—such as pharmacists, social prescribers, physiotherapists and health coaches—to offer a broader range of services and better coordinate care for patients in the area.
